PRIVACY POLICY

Privacy policy

We, SRL Medisearch Inc. believes it is its social responsibility to handle personal information including specific personal information properly under the Privacy Policy given below. We intend to do our best to protect such information by improving the system and management method for the protection of personal information.

1. Observance of laws and regulations

We comply with laws, regulations, and executive order regarding the proper handling of personal information including the Act on the Protection of Personal Information and the Act on the Use of Numbers to Identify a Specific Individual in the Administrative Procedure.

2. Proper handling of personal information

We specify the purpose of use to collect, use, disclose personal information in a proper and fair manner within the limits of necessity in connection with the execution of our services. We handle personal information to be collected within the scope of purpose of use and take measures to prevent any such information from being used for any other purpose.

3. Safety measures

Our policy is to take preventive measures against the forecasted hazard to which the personal information is exposed, such as illegal access, loss, destruction, alteration and leakage and to take suitable countermeasures should an incident or accident occur.

4. Request for disclose and complaints handling

We faithfully and swiftly correct, add, delete or suspend corresponding information based on verification of the request.

Disclosure Pursuant to the Act on the Protection of Personal Information

SRL Medisearch Inc. (the “Company”) makes the following disclosure concerning the handling of personal information (here and hereafter, excluding specified personal information) provided by customers and business partners in accordance with the Act on the Protection of Personal Information.

1. Purposes of use of personal information

(1) The personal information of customers and others in the possession of the Company and the purposes of use of that information are as set forth below:

2. Joint use of personal information

The Company jointly uses personal information with the specified persons indicated below. The purposes of use of personal information handled in such joint use are to the extent of the details set forth in (1). Further, SRL Medisearch Inc. is responsible for management of such personal information data.

3. Third-party provision of personal information

The Company properly manages personal information acquired from customers and others and does not provide personal information to third parties without obtaining the prior consent of the relevant individual. However, this does not apply in the following cases:

  • Cases where personal information is provided to a business service provider or other party under appropriate supervision by the Company in order to achieve the purposes of use
  • Cases where, within the scope of the purposes of use, personal information is provided to a joint user regarding which prior notice was provided to the relevant individuals or a public announcement was made
  • Cases pursuant to laws and regulations
  • Cases where there is a need to protect human life, limb, or property and obtaining the consent of the relevant individual would be unfeasible
  • Cases where there is a particular need for the enhancement of public health or the sound development of children and obtaining the consent of the principal would be unfeasible
  • Cases where there is a need to cooperate with a central government organization, a local governmental body, or a person entrusted by such an organization or body when performing matters prescribed by laws and regulations and there is a possibility that obtaining the relevant individual’s consent would interfere with the performance of those matters

4. Matters relating to requests for disclosure, etc.

The Company shall respond as set forth below to requests from the relevant individual or the agent of the relevant individual for disclosure, revision, termination of use, or the like (referred to as “Disclosure, etc.”) of the personal information of a customer or other person in the Company’s possession:

(1) Information subject to requests for Disclosure, etc.

The subject matter of personal information subject to Disclosure, etc. shall be as set forth below.

  • Name
  • Address
  • Sex
  • Email address
  • Telephone number

(2) Where to submit requests for Disclosure, etc.

Please make requests for Disclosure, etc. by submitting the specified request form with the necessary documents to the following address by delivery-certified mail. Please write in red ink “Request for Disclosure, etc.” on the envelope.

(3) Documents to be submitted when making a request for Disclosure, etc.

When making a request for Disclosure, etc., download request form (A), fill in all of the required information, attach a document specified in (B) to verify your identify, and submit that application to the address specified in (2) by delivery-certified mail.

(A) Request form specified by the Company
■ Request for Disclosure of Personal Data in Possession
■ Request for Revision of Personal Data in Possession
■ Request for Prohibition of Use of Personal Data in Possession

(B) Documents verifying identity

■ Copy of a driver’s license, passport, or other official document (1 document)

(4) Request for Disclosure, etc. by an agent

When the person making a request for Disclosure, etc. is the legal representative of the relevant individual or an agent appointed by the relevant individual in relation to the request for Disclosure, in addition to the documents specified in the preceding paragraph, enclose a document specified in (A) or (B) below and a document specified in (C) to confirm the identity of the representative or agent.

(A) In the case of a legal representative
■ A copy of the family register of the relevant individual or certificate of registration of guardian of adult

(B) In the case of an agent
■ The power of attorney specified by the Company and the relevant individual’s certificate of seal impression

(C) Documents verifying the identity of the representative or agent
■ Copy of a driver’s license, passport, or other official document (1 document)

(5) Fees for a request for Disclosure, etc. and method of payment

The fee is 500 yen per request.
Please enclose 500 yen of postage stamps with the request documents.
If the full amount of the fee is not paid or the fee is not enclosed, the Company will notify the requesting party to that effect, but if payment is not made within the specified period, the Company will consider that the request for Disclosure, etc. was not made.

(6) Method of responding to requests for Disclosure, etc.

The Company will respond in writing to the requesting party’s address indicated on the request form.

(7) Purpose of use of personal information acquired in relation to requests for Disclosure, etc.

Personal information acquired in conjunction with requests for Disclosure, etc. shall be handled only to the extent necessary to respond to the request for Disclosure, etc. Submitted documents shall be retained for two years from the completion of the response to the request for Disclosure, etc. and then shall be destroyed.

(8) Cases where requests for Disclosure, etc. shall not be complied with.

In cases where a request for Disclosure, etc. falls under one of the following grounds, the Company shall not comply with the request. If the Company does not comply with a request, the Company shall provide notice to that effect with an indication of the grounds. Capping cases where the Company does not comply with a request, the specified fee shall not be returned.

  • Cases where the address stated on the request form or the address stated on the documents submitted to confirm identity do not match with the address in that the Company’s records or the Company is otherwise unable to confirm the applicant
  • Cases where a request is made by a representative and the Company is unable to confirm the representative
  • Cases where there is a deficiency in the request documents or otherwise
  • Cases where the subject matter of a request for Disclosure, etc. does not correspond to any of the subject matter specified in Item 3(1) of the Disclosure items
  • Cases where complying with a request would pose a risk to human life, limb, property, or other rights and interests of the requesting party or a third party
  • Cases where there is a likelihood of substantial impediment to the proper implementation of the Company’s business operations
  • Cases otherwise contrary to laws and regulations
